耿直 describes a person's honest, straightforward character, often to the point of bluntness — it suggests an integrity that won't bend to social pressure. Compared to 坦率 (frank in speech) or 坦诚 (open, candid), 耿直 emphasizes a principled directness that can sometimes offend; it's stronger than the simple 直 (frank).
