Definition
神经病 literally means 'neurological disorder' (a medical condition), but in everyday Chinese it's far more common as a colloquial insult meaning 'crazy person' or 'lunatic'. Using it to call someone crazy is very strong and offensive—stronger than 疯了 or 精神病 in casual insults. Always be aware of the context: medical vs. derogatory.
