失实 is a separable verb (离合词) meaning 'to be inaccurate, to stray from the truth', typically of reports, news, or data. 失 and 实 can split when inserting aspect markers (了, 过) or measure words: 失了实 ('became inaccurate'), 失过实 ('has been inaccurate before'). It is often used attributively before 的 in formal writing.
