出人命 is a verb-object phrase meaning 'to cause a death' or 'to result in someone's death'. It is commonly used when describing accidents, incidents, or situations that turn fatal. As a separable verb (离合词), the verb 出 can be separated from the object 人命 by aspectual particles (了, 过) or measure words — for example, 出了人命 ('a death occurred') or 出过一次人命 ('a death happened once').
